Media Consent

1.) I hereby grant to Calista Corporation; its subsidiaries, affiliates, agents, and others working for them or on their behalf and their respective licensees, successors, and assigns (herein collectively called the “licensed parties”) the absolute right and permission to use, publish, broadcast, and copyright my voice recording, name, picture, and likeness, or any material based upon or derived therefrom, or to refrain from so doing, in any manner or media whatsoever for purposes of advertising or trade in promoting and publicizing Calista Corporation; its subsidiaries and affiliates.

2.) I agree that any voice recording, picture, or likeness of me, or anything derived therefrom created by the licensed parties is owned by them. If I should receive any print, negative, or other copy, I shall not authorize its use by anyone else.

3.) I shall have no right of approval, no claim to additional compensation, and no claim (including, without limitation, claims based upon invasion of privacy, defamation, or right of publicity) arising out of any use, alteration, distortion, or illusionary effect or use in any composite form.

4.) I agree that this release does not in any way conflict with any existing commitment on my part.

5.) I acknowledge that: (a) I have not received and will not receive any money or other consideration or compensation for giving this consent and release unless otherwise noted; (b) I am an adult, 18 years or older; and (c) I have read and understand this form.
