Each option below is one entry. If a Calista Shareholder has already completed one of the options (for example, already has direct deposit), the Shareholder automatically receives one entry. The five entry options are as follows:

• Opt-in for Direct Deposit
• Opt-in for electronic delivery of Annual Meeting materials
• Opt-in for electronic delivery of the Storyknife newsletter (an option in your MyCalista account)
• Complete a Stock Will Form (Class A and B shares only)
